Remodel plans get drawn around where fixtures look right and then meet the reality of where the drains are. Moving a sink a few feet is minor. Moving a toilet or a shower drain means opening the floor and re-running waste with the correct fall, and finding that out after the tile is ordered is an expensive way to learn it.

Usually yes, but it is the most expensive fixture move because waste lines need consistent fall to drain properly. That often means opening the floor and re-routing beneath it. Sinks are far easier to relocate. If budget is a constraint, keeping the toilet and shower drains where they are and moving everything else is where the savings live.
When should the plumber be involved?
Before the layout is final, and well before demolition. The existing drain routing determines which layouts are straightforward and which are structural, and knowing that while the plan is still on paper is the difference between choosing a constraint and discovering one.
Do remodel plumbing changes need a permit?
Relocating drains or supply lines generally does, while a like-for-like fixture swap generally does not. Permitted work is inspected, which also protects you if the house is sold later.
