Because the device can fail without any outward sign. Its internal checks and relief valve wear, and a failed device keeps passing water normally while no longer preventing backflow. An annual test is the only way to know it is still doing its job, which is why water purveyors require it rather than leave it to judgement.
What actually is backflow?
It is water flowing the wrong way through the supply, back toward the drinking water system. It happens when pressure drops, for example during a main break or heavy draw, and the resulting suction pulls water back through any connection. If that connection is an irrigation line or a boiler, what gets pulled back is not water you want to drink.
